Here's the hardware maintenance manual for the laptop.
Go to p.42 to view the procedure to remove the fan and heat sink assembly. It also details the amount of thermal paste to apply etc.
You can check if the fan needs cleaning and also if it appears noisy (bearing?). If you decide to replace the fan the part (fru) number is on p.71 Item #7 for Fan C 80X8 i.e. 5F10N67388. Search online using the part number only to find suppliers that suit you best.
If after refreshing the thermal paste and cleaning/replacing the fan there are still heat issues is the fan always on high speed? if so then try starting the laptop in safe mode and check if it now OK. If so it may be a startup program that is causing the problem - (check in Task Manager > Startup, Windows in normal mode) or a driver problem.
For performance issues, check in Event Viewer for any the same Critical, Error or Warning events that are repeatedly being logged as they may indicate what's happening.

What is the model number of the laptop?
Is the fan vent clean and clear of any obstructions?
Have you thought about refreshing the thermal paste on the CPU? After 6+ years it may have dried out by now and is less effective at heat transfer from the CPU to the heat sink.
Is the fan sounding OK - not noisy etc?

@jayeff It's an IdeaPad flex 5-1470. I haven't taken the heatsink off so I don't know about the thermal compound. I haven't checked the fan either. The fan is a bit noisy and makes a whistling sound.
