Random issues all over. We did replace the screen about 6 months ago, but the issues have been happening before and after the repair. One- Prox sensor sometimes works, sometimes doesnt. Phone will power off randomly, apps like Snapchat seem to trigger it more. And the most annoying issue is that 75% of the time, it takes multiple presses to get the power and home buttons to work. Very frustrating. Help??

The screen is most likely not installed properly especially in terms of adhesive. You may find that the proximity sensor works perfectly if you push the screen down at the top especially near the proximity sensor.
To fix that adhesive probably has to be redone for the screen.
Also power off issue is most likely caused by a bad battery.
Also multiple presses to get power / home button to work is maybe either software hanging up due to some error caused by apps or etc or physically damaged home / power button during repair.
