I am having the same problem with EOS-5D Mark II upgrading from firmware 2.0.9 to 2.1.2. Tried multiple cards some previously formatted and used with the camera and others freshly formated with fat32 using both windows and MacOS downloads of 2.1.2 formware.
Did you ever get this resolved and what was the underlying issue? Camera was working fine until the upgrade attempt.
