Where can I purchase a replacement screen and how do I replace it?
Mine is from Cricket, though...not T-Mobile.

Trust me, don't waste your money by diy. Bring to local phone repair shop.
Replacement screens from ebay will be poorer quality then in that shop
And by looking at your question i asume there is no experience with this type of repair... its not the best way to start from ...
