Your problem has nothing to do with the camera. The camera saves your images on the SD card in folders which are named 100NIKON, 101NIKON and so forth. Image files are given a sequential number from 0000 to 9999, and only when the file number rolls around from 9999 to 0000 a new folder is created on the card.

When using a windows PC to read the card, this is the folder/file structure you should see on the card.

However, when you copy the images to the Pictures folder on your computer, Windows tries to be helpful and creates so-called virtual folders by date - which can be rather annoying if you don't want this. This behavior can be defeated: in the folder window there's a dropdown button labelled Arrange by:, set this to Folder and the virtual folders that classify your pictures by date will disappear.
