Just replaced the headphone flap a few minutes ago - use a typical hair pin (the blade should be flat enough and narrow enough) and gently insert it on the hook side of the tap. Then gently wiggle the flap while you pull and it should come out.

To insert the new tap use a pair of tweezers to grab the tap just about the hook. Push it down into the phone's slot but be gentle, otherwise you might deform the tap. And in it goes. The whole procedure took maybe 5 minutes.
